- Make an appointment by clicking the “Schedule a Visit” tab below which will take you to our secure Patient Portal through AthenaNet where you will need to create your personal log on.
- When scheduling your first visit, you will be encouraged to complete your past medical history, social history and family history through the Portal.
- You will also need to go to the “My Health” Tab on the Portal, and click on “Medical Forms”. Three forms will need to be reviewed and signed prior to your first visit. You can either print these forms and sign them or review and sign them electronically (preferred). Until we are able to take Medicare, if you have Medicare, print out and sign a Private Physician Contract which allows you to be seen by Dr. Chapman. See the Medicare tab for further information.
- If you have printed and signed the above forms, be sure to bring them with you to your first visit to be reviewed with Dr. Chapman and scanned into your confidential digital medical record.
